There is a persistent cultural shorthand in how we talk about hormones and gender. Estrogen, we are told, belongs to women. Testosterone belongs to men. The story is tidy, which is probably why it has survived so long — even as the biology, examined more closely, tells a considerably more interesting tale.

Testosterone is not a male hormone. It is a human hormone, present and functionally essential in people of every biology. Women produce it — in the ovaries and adrenal glands — throughout their lives, and the research emerging over the past decade is making a convincing case that when those levels fall below a woman's personal optimal, the consequences are quieter than a hot flash, harder to name, and easier to dismiss. Which is precisely why they go unaddressed for so long.

What Androgens Actually Do in the Female Body

In women, testosterone operates across a surprisingly broad physiological territory. It plays a role in maintaining skeletal muscle mass, supporting bone density, regulating fat distribution, and modulating the cardiovascular system. It is involved in mood stability, cognitive sharpness, and the kind of low-level motivational drive that doesn't have a medical name but that most people recognize immediately when they lose it: the sense of being engaged in your own life.

Libido gets most of the clinical attention when androgens in women are discussed, and that attention is warranted — the evidence linking testosterone to sexual function and desire in women is among the most robust in the field. But reducing the conversation to libido alone is a little like describing a symphony by mentioning only the percussion. The fuller picture is more interesting.

More recently, a position statement from the Latin American Association of Gynecological Endocrinology examined the evidence around androgen therapy in midlife and older women and concluded that the data supports clinical consideration — not as a fringe intervention, but as a legitimate dimension of hormonal care (Pilnik et al., 2025).

These are not outliers. They are part of a growing body of literature quietly repositioning how clinicians think about female androgen health.

Why the Decline Goes Unnoticed

Women's testosterone levels begin declining in their 20s — earlier, in fact, than the estrogen decline that typically defines the conversation around menopause. By the time a woman reaches her 40s, her androgen levels may be roughly half what they were in her late teens. This is a gradual erosion rather than a sharp drop, which makes it easy to normalize — to attribute the fatigue to a busy schedule, the mental fog to stress, the flattened motivation to circumstance.

The absence of a dramatic hormonal event, like the estrogen shift of menopause, means there is rarely a clear before-and-after moment. There is just a slow, almost imperceptible narrowing of vitality. Energy that used to be available without effort. Resilience that used to feel effortless. Clarity that used to arrive without caffeine or willpower.

What complicates this further is the way standard laboratory ranges are constructed. A testosterone result that falls within the broad population "normal" range may still be well below the level at which that particular individual feels and functions optimally. The reference range describes a population. It does not describe a person.

A thoughtful hormonal assessment for women looks at:

  • Total and free testosterone, not just one measure
  • DHEA-S, which is a key androgen precursor produced by the adrenal glands
  • Estradiol and progesterone, since androgens do not operate in isolation
  • Sex hormone binding globulin (SHBG), which determines how much testosterone is biologically available
  • Thyroid function and cortisol, because the endocrine system is a web, not a series of isolated levers

When the Conversation Finally Happens

What is striking, when women do begin addressing suboptimal androgen levels with clinical support, is how often they describe the improvement not as feeling different, but as feeling like themselves again. That language matters. It suggests that what was lost was not an enhancement but a baseline — a normal functional state that had eroded so gradually it had simply come to feel like the new normal.

This is perhaps the most underappreciated cost of leaving women's androgen health out of the mainstream hormonal conversation for so long: not a dramatic decline, but a slow, quiet dimming that becomes indistinguishable from ordinary aging. The evidence suggests that it is neither inevitable nor, for many women, irreversible. That is worth knowing.

The biology was never as simple as the shorthand suggested. It rarely is. But in this case, the more accurate picture happens to be the more hopeful one — and that, too, is worth sitting with.
